Please Note, We will be closed on a Tuesday from 12pm on the following dates, for mandatory staff training: 7th September 21, 12th October 21, 9th November 21, 11th Jan 2022, 1st Feb 2022 & 8th March 2022.

If you require help or advice when we are closed, please still ring the surgery number (0115 9861128) and you will be transferred to our Out of hours provider who will help you get the correct treatment or advice. alternatively you can also dial 111 or attend the Urgent Care Centre, located on London Road.

REMEMBER A&E is not for minor ailments but for EMERGENCIES ONLY.

New Patient Registration

Our lists are currently open and we are welcoming new patients.

You can register at reception by using a registration form and supplying us with proof of identity and address.

Any patients registering who have come from outside of the UK must provide proof of entry, proof of a visa or home office letter.

All new patients will be asked to attend a new patient health check with the practice nurse or a healthcare assistant. This will be arranged when registering. 

Change of Details

We must ask that you notify us of any change to your address or telephone numbers. We can only use the current details and if they have not been updated then we may not be able to contact you when necessary. We do use hospital letters to keep updated on addresses and if they differ from our details you may be asked for up to date proof. If you move out of our practice area you may be asked to register at a doctor closer to your new home. You will be given 28 days to do this.
